Register online using the secure NHS “Register with a GP surgery” service. This service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Once we receive your completed forms, we will contact you within five business days to schedule your new patient appointment. Please bring the following to your appointment:
• One form of photo ID (passport, driver’s license, or birth certificate)
• One form of address ID (utility bill, rental agreement, or bank statement—dated within the last 3 months)
• Your completed New Patient Questionnaire
At your appointment, we will review your information, conduct a health check if appropriate, and answer any questions. You will be fully registered once it’s complete.
